Seven Healthcare are seeking an experienced Pharmacist for a contract-to-hire position in Oklahoma City. This role involves the evaluation and monitoring of drug therapy for home care patients, working closely with physicians, nurses, and other healthcare professionals. Pharmacists will be engaged in a dynamic environment with on-call shifts to ensure comprehensive patient care.

Key Responsibilities of the Pharmacist

• Evaluate and monitor drug therapy for home care patients
• Adhere to organization policies and applicable laws/regulations
• Coordinate referrals and admissions with healthcare professionals
• Verify new patient orders and facilitate documentation
• Collaborate with healthcare team in patient care planning
• Conduct assessments and document drug-related information
• Monitor drug therapy outcomes and provide consultations
• Process and verify medication and supply orders
• Compound sterile and non-sterile mixtures while adhering to standards
• Oversee Pharmacy technician functions

Requirements of the Pharmacist

• Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred
• Registered Pharmacist License in state(s) of practice
• Minimum of two years of recent experience in an acute-care setting or one year in home infusion therapy
• Experience with compounding infused drugs
• Strong organizational and communication skills
• Flexible schedule and availability for on-call requirements
